Do not alter the page layout or the number of pages of the designated forms. Ensure content fits within the original pages. 3. In the "Current Affiliation/Address" section, write your current status at the time of writing (prior to entering the Master's program). 4. In the "Educational Background" section (starting from elementary school), list your history in chronological order (oldest to newest), including your university faculties/departments. Example: 〇〇 University, Graduate School of 〇〇, Research Student (Country Name) 5. For the GPA, calculate your score according to the attached "GPA Calculation Method" sheet and enter the value. *If a GPA is already printed on your transcript, and it is out of a 4.0 scale, you may use it even if the calculation method differs from the attached sheet.
