１．学位記伝達式へ出席する場合
伝達式で学位記及び証明書を授与します。（9月25日16時～）東北大学学位記授与式及び生命科学研究科学位記伝達式について・About the TU Commencement Ceremony and the GSLS Graduation Ceremony (Degree Conferment Ceremony)
２．学位記伝達式へ出席しない場合
学位記及び証明書の受領方法を(1)～(3)より選択してください。
(1)教務係窓口で受け取る。（9月26日～9月30日）
生命科学研究科教務係窓口で受領してください。

受取時間は8：30～12：00、13：00～17：00とします。
(2)研究室で受け取る。（9月26日以降発送）
所属研究室へ発送しますので、受領してください。

レターパックプラス（600円）＊へ「郵送先住所・氏名」を記入し、9月11日(木)までに教務係へ提出してください。送料は修了者負担となります。

＊母国へ帰国する方は、学位記等を受理してから帰国することにご協力をお願いします。
３．修了証明書等が複数枚必要な場合
修了証明書・成績証明書が複数枚必要な場合は、回答フォームより追加分を申し込んでください。
追加分の証明書は、学位記と一緒にお渡しします。
４．学生証の返却（いずれかの方法で必ず返却してください。）
・伝達式に出席する場合：会場で返却
・教務係窓口で学位記を受領する場合：窓口で返却
・研究室で学位記を受領する場合：研究室へ返却
・仙台を離れる場合：離仙前に教務係窓口に返却するか下記住所宛てに郵送

For those who do not respond and are not present at the GSLS graduation ceremony, diplomas will be sent to the respective student's laboratory after March 26.

Please select the method of receiving your diploma and certificates from the options below (1)～(3):

You will need the "Letter Pack" envelope. Please write your name and mailing address on the Letter Pack Plus (600 yen, red one)* and submit it to the Academic Affairs Section by Thursday, September 11. Graduates are responsible for postage.

*We request the cooperation of those who will be returning to their home countries to receive their diplomas, etc. before leaving Japan.

4. Returning your student ID card (Please be sure to return your ID card by one of the following methods.)
● If you attend the ceremony: Return your ID card at the venue.
● If you pick up your degree at the Academic Affairs Section: Return it at the office.
● If you pick up your degree at your laboratory: Return it to the laboratory staff.
● If you are leaving Sendai: Return it to the Academic Affairs Section before you leave Sendai or mail it to the address below.
